The map used in the article is a reconstruction of one that appears on page 79 of Craig Stephensons PhD The Voluntary Maternity Hospital: a social history of provincial institutions,with special reference to maternal mortality, 1860-1930. The original source data is given as Burdett's Hospital Yearbook, 1894-1930 . I do not have access to this source and have rebuilt it using a number of different sources. There may be omissions and the long/lat are all approximations. Any error in the list is entirely mine.
